Our responsibilities include

  • Assessments

    Advanced assessments, diagnosis and early treatment of acute and chronic illnesses

  • Diagnosing

    Initiate and order diagnostic imaging, pathology and provide referrals

  • Medication Management

    Prescribe, deprescribe and rationalise medications. Monitor for polypharmacy and review psychotropic medications. Provide immunisations.

  • Post-hospital care

    Streamlining the transition into aged care, support with admitting residents, prescribing medications, pain management and complex health needs

  • Collaboration

    Collaboration with GPs and all health professionals. Family liaison and case conferences.

    Consultancy and support for the facility in exceeding the Aged Care Quality Standards

  • Wound Management

    Management of chronic and complex wounds (including vac dressings, debridement & compression bandaging)

  • Education

    Mentoring, up-skilling and evidence-based education for nursing staff.

    Common topics may include; wound management, medications, infection control, falls management, dementia/delirium care, continence care

  • End of Life and Advanced Care Planning

    In collaboration with GPs, Specialists and local palliative care teams, we support the implementation of advance care planning and end of life care.

FAQs

What is the referral process?

The first step is to book an initial phone consultation. From here, we will discuss the service options available and arrange a time to visit. Please note, Call Georgia is not an emergency service and we are only able to review patients from 8am to 6pm (Monday to Saturday).

Who can refer?

We accept referrals from anyone. We try to liaise with existing care providers to collate as much information as possible.

How much will it cost?

The cost is variable for each patient depending on the service required and location. To give you an idea, an initial (comprehensive assessment) can range from $250 to $350. The cost for follow-up visits will be negotiated on a case-by-case basis. A Medicare rebate may be available for this service. Fees for residents within RACFs will be negotiated with the provider.

What area do you service?

We cover Sydney City, Eastern Suburbs, Lower North Shore and the Inner West. In some cases, we will travel further for a review but fees may apply for additional travel.
